EUSA POLICY - THE LEAGUE WILL NOT ACCEPT LATE OR MAIL-IN REGISTRATION.
YOU MUST SIGN UP IN PERSON AT ONE OF THE SIX SESSIONS LISTED BELOW

Bring copy of birth certificate or other proof of birth date for all players new to our league. Payment must be by check or money order (cash will NOT be excepted).

Sign up for the fall 2003 season at one of the following “in person” registrations ONLY:

Minnie B. Veal Recreation Center, 1070 Grove Ave (just south of Inman Ave)
Friday, May 2, 2003, 7 pm to 9:45 pm
Saturday, May 3, 2003, 2:30 pm to 5 pm

Stelton Community Center, 328 Plainfield Ave (between Edison Train Stn and Rte 27)
Friday, May 16, 2003, 7 pm to 9:45 pm
Saturday, May 17, 2003, 3 pm to 6 pm
Friday, May 30, 2003, 7 pm to 9:45 pm
Saturday, May 31, 2003, 3 pm to 6 pm

Recreation League Fees: (Check or money order - NO CASH)
For Edison or Metuchen residents: $40 for each player registering

For non-residents: $50 for each player registering

Players who are new to the league must present a copy of their birth certificate as proof of date of birth.

At registration you will be asked to fill out the application completely and make payment by check or money order only, made out to “EUSA” (CASH CANNOT BE ACCEPTED). You will also be asked to review the League's Rules of Conduct and to acknowledge your acceptance of them. Each family will also be required to select one of the volunteer categories in which the member is willing to contribute some of their time to the league. Coach and Assistant Coach volunteers will be required to fill out a separate Coach's Application.

COACHES:
If you would like to apply to coach in EUSA, you must complete a coaching application which will be available at the in-person registration sessions. On or about the time of the sessions, the form may also be available for download and printing on this page of our web site. All EUSA coaches and assistant coaches are required to have completed an F License course.

REFUND POLICY:
All refund requests must be in writing. Include the player's full name, grade, and name of school. Enclose a copy of both sides of your cancelled check, and mail to:

In general, refunds will have a $15 administrative fee deducted. Refund requests received after the start of the season may or may not be granted, depending on the circumstances.
