EUSA Incident Report:

This form should be used to record the occurrence of injuries which are not considered minor. It should also be used to report incidents of inappropriate behavior, physical violence, verbal abuse, or any actions which are thought to be serious enough to be brought to the attention of the league administrators for review and disposition. This report must be signed and it must be submitted no later than seven days after the date of the incident in order for it to be considered valid.

"KIDSAFE" Volunteer Disclosure Form:

All coaches and assistant coaches are required to complete and submit this form to the league Kidsafe coordinator. Must be renewed annually or whenever requested by the league.

New Jersey Youth Soccer Membership Application:

Must be completed be each player signing up to play on a Travel team. Requires signature of one parent and the player.

Medical Release:

Must be completed and NOTARIZED for each player signing up to play on a Travel team.

Sportsmanship Pledge (S.A.G.E. form) for Coaches:

Must be read and signed by each coach and assistant coach on a Travel team.

Sportsmanship Pledge (S.A.G.E. form) for Parents:

Must be read and signed by BOTH PARENTS OR GUARDIANS of each player on a Travel team.

Sportsmanship Pledge (S.A.G.E. form) for Players:

Must be read and signed by each player on a Travel team.
